How many victims? About 7,600.

What type of personal information? Names, addresses, and Social Security numbers.

What happened? A CMS employee disclosed the personal information to an outside contractor prior to obtaining appropriate authorization.

What was the response? CMS has worked collaboratively with the contractor to ensure information extracted from the applications was destroyed. CMS is taking measures to prevent any additional information from being released. All affected individuals are being notified.

Details: The contractor, following a work order, used the application information to create a CMS database of applicants for a specific learning community. CMS revoked the contractor's access to the CMS database.
