Approximately 43,000 incident reports and 13,000 payroll reports were compromised in a breach of a North Dakota Workforce and Safety Institute (WSI) server, after the North Dakota Information Technology Department (ITD) detected unusual activity on a server.
The reports contained personal identifiers including social security, names, employer and medical information however only employee and worker reports filled online between 2006 and 2013 may be at risk, according to a release.
After noticing the activity, the ITD contacted its federal partner, the Multi-State Information Sharing and Analysis Center, to further test the server. The WSI was notified on June 10.
Authorities said currently there is no evidence that any of the information was moved or duplicated from the server. WSI has set up a call center and is providing a year of free identity repair services to affected individuals.
