Jameelah Jossiah, 24, a former flu clinic billing clerk at the Kanawha-Charleston Health Department in West Virginia copied down personal information of patients and used it to make a $400 purchase at a local Wal-Mart.
How many victims? When police searched Jossiah’s house they found a handwritten list of about 14 victims.
What type of personal information? Names, social security numbers, addresses and other unspecified personal information.
What was the response? As a precautionary measure, the agency is alerting all 11,000 people who had flu shots during the month of October 2008. Jossiah was arrested last Friday and was released on a $10,000 bond. A preliminary hearing is set for Thursday.
